To support member adaptation of value-based care best practices and change strategies, we are excited to announce the launch of the AACVPR Value-Based Care “Session-in-a-Box” presentation. This slide deck will provide meeting planners with a standard set of slides to introduce value-based care and educate on the application of value-based care principles. Slides include comprehensive scripting and guidance to help facilitators educate on these essential principles.
As you begin preparing for your 2024 educational offerings, the AACVPR Value-Based Care Committee encourages you to consider incorporating this presentation into your conference agendas. The “Session-in-a-Box” is available for free download, and may be given by any person comfortable with the slide deck and value-based care principles. Change Packages
In 2018, AACVPR partnered with Million Hearts and the Center for Disease Control (CDC) to create the Cardiac Rehabilitation Change Package. A second edition was released in 2023. In 2019, Million Hearts and the CDC released the Tobacco Cessation Change Package.
Cardiac Rehabilitation Change Package
The CRCP was developed in partnership with Million Hearts® to present a listing of process improvements that CR champions can implement as they seek optimal CR utilization. It is composed of change concepts, change ideas, and tools and resources. The purpose of the CRCP is to help quality improvement teams from hospitals and CR programs implement systems and strategies that target improved care for more patients eligible for CR. The CRCP was originally published in 2018 and was used by hospital and CR program staff to increase CR participation for five years. The second edition, published in 2023, includes new change concepts, change ideas, and twice as many tools and resources that reflect the latest evidence and experience of hospitals, CR programs, and national organizations and quality improvement organizations.

Hypertension Control Change Package
The Hypertension Control Change Package (HCCP) was originally published in 2015 and has been used in the field to improve hypertension (HTN) control by a variety of health centers and clinics. New clinical guidelines, development of new resources, and general advances in quality improvement for HTN management prompted the need for this updated version. The HCCP presents a listing of process improvements that outpatient clinical settings can implement as they seek optimal HTN control. The HCCP is composed of change concepts, change ideas, and evidence- or practice based-tools and resources. The HCCP is broken down into four main focus areas: key foundations, equipping care teams, population health management, and individual patient supports.

Tobacco Cessation Change Package
The Tobacco Cessation Change Package (TCCP) is a quality improvement tool created by the Centers for Disease Control and Prevention (CDC) that is intended for health care professionals in outpatient, inpatient, and behavioral health settings and public health professionals who partner with these groups. The TCCP presents a list of process improvements that clinicians can implement as they seek to deliver optimal treatment to patients who use tobacco. The change package also gives clinical teams a practical resource to increase the reach and effectiveness of tobacco cessation interventions and to incorporate these interventions into the clinical workflow. The TCCP is composed of change concepts, change ideas, and tools and resources, and has five focus areas: key foundations, equipping care teams, screening, treatment, and referral and follow-up.
