The discharge date must fall within the data collection period. The patient’s enrollment date and their referral date do not need to fall within the data collection period.

The enrollment date must fall within the data collection period. Discharge date is not assessed for this measure.

PR Functional Capacity, Dyspnea, and Health-Related Quality of Life Measures: Patients will be included or excluded from the measure calculations in the registry based on their DISCHARGE date. Enrollment in Cardiac Rehab
CR & PR Enrollment Measure: Patients will be included or excluded from the measure calculations in the registry based on their REFERRAL DATE. To qualify for the numerator of the measure, the patient must complete their index visit (ENROLLMENT DATE) within the data collection period. If the index visit takes place after the data collection period (i.e. starting in January 2026), they will be excluded from the calculations.
Enrollment in Pulmonary Rehab
CR & PR Enrollment Measure: Patients will be included or excluded from the measure calculations in the registry based on their REFERRAL DATE. Adherence to Cardiac Rehab
CR & PR Adherence Measure: Patients will be included or excluded from the measure calculations based on their ENROLLMENT DATE, and DISCHARGE DATE – these dates need to be within the data collection period for them to be included in the calculations. Patients that enrolled prior to the data collection period or completed/dropped out of the program after the end of the data collection period will be excluded. To qualify for the numerator of the measure, PR patients must have completed at least 10 sessions, and CR patients must have completed at least 12 sessions.

CR & PR Adherence Measure: Patients will be included or excluded from the measure calculations based on their ENROLLMENT DATE, and DISCHARGE DATE – these dates need to be within the data collection period for them to be included in the calculations.
